Responsibilities include the coordination
and case management of patient care, home
visits for clinical assessments and
provision of direct care.
Opportunities for training in point of
care/computer documentation and TeleHealth

Per Diem Visiting
Nurse position - Milford P.A.
|
Responsibilities include the coordination
and case management of patient care, home
visits for clinical assessments and
provision of direct care.
Opportunities for training in point of
care/computer documentation and TeleHealth
technology. |
- Must have excellent
communication, organizational and time
management skills.
- Current NJ and PA
state RN licenses.
- Minimum 1 year acute
care experience required.
- Prior home care
experience a plus.

Telehealth Nurse reporting out of their
Sparta, NJ office. Must have strong
clinical, communication and organizational
skills.
Duties include:

Oversight of daily
telemonitoring program

Assessment of
telemonitoring data and necessary
clinical interventions

Assist in development of
plan of care
-
Collaboration with nurse
case managers, supervisors and
physicians
Current NJ and PA
RN State Licensure and experience in
home care required.
