With this program, your care team receives daily reports about your
health. Home-based monitoring will enable a nurse to view a
status report on your health on a more consistent schedule and more
frequently than your regularly scheduled visits to the doctor's office.
Daily information serves as a useful supplement, not a
replacement, to the care you get from your doctor.

Skilled Nursing Our highly trained and competent
Registered Nurses provide skilled nursing evaluations, assist with

Home Health Aide Our certified Home Health Aides
provide support with every day living and assist with tasks such as
bathing, dressing, meal preparation and light housekeeping. A care plan
is developed to meet each patient's individualized needs.
Occupational Therapy Our licensed Occupational Therapists provide
teaching and exercises designed to restore independence in performing
daily activities. This is often accomplished through the use of adaptive
equipment for bathing, grooming and cooking.
Physical Therapy Our licensed Physical
Therapists use exercises targeting specific parts of the body to
strengthen, regain motion, and increase movement following an injury or during illness.
Speech Therapy Our licensed Speech Therapists'
primary goals are to improve communication skills via the spoken word as
well as listening and writing. In addition, exercises are used to
improve swallowing after a stroke or while in the midst of an illness.
Social Worker Our licensed Social Worker provides
short-term counseling services, referral to and coordination with
community resources and assistance with living arrangements, finances
and long-range planning.
Registered Dietician
Our licensed Registered Dietician provides nutritional assessment and education for you and your family to
meet your dietary needs.
